The Bruce Highway will remain closed for hours after two ambulances and three trucks were caught in a pile-up near Gladstone in central Queensland.

A B-double truck crashed on the highway about 2am, which caused two other B-doubles to collide with it.

One truck driver is still fighting for life in hospital, while two people are in a stable condition.

It's understood conditions were foggy and smoky, amid hazard reduction burns nearby.

Two ambulances responding to the crash then also collided at the site.

A witness said one of the trucks had tried to reverse off the road, forcing the ambulances into the embankment.

The drivers of the trucks were trapped before being cut free.

One driver, a man in his 50s, was taken to hospital with life-threatening injuries.

The other two drivers, both men in their 20s, were taken to hospital in a stable condition.

9News understands the paramedics in the ambulances were unhurt and were able to provide help at the scene.

The Bruce Highway is closed between Gin Gin and Miriam Vale, a stretch of about 100km.

Traffic remains well backed up with drivers urged to avoid the area.

One driver told 9News he had been warned the wait could be as long as 15 hours.

"Word on the street is that it could be another 15 hours," Christian Murray said.
